Please feedback any issues or concerns you think the CCG should know about, including actions you have taken to address the concern or issue. Feedback will assist the CCG in commissioning better services from the local service providers (eg. issues with local NHS services or Care Services such as care homes or domiciliary care services).
Please do report, as the volume of information gathered will assist in identifying “problem” areas for practices. If you have a persistent problem, the likelihood is that so does a neighbouring GP practice.
